MOSSA Extension Request Form


Completing the video assessment process within 90 days allows you to:

  • Remain on Autoship for your program.
  • Receive access to the most current workouts, materials, education, resources, and more.
  • Stay on track with your facility’s instructor team.
  • Continue receiving your releases at the discounted Autoship rate.
  • Officially begin your journey as a certified instructor.

If you are unable to submit your assessment video within 90 days, a one-time* 30-day extension may be requested. All extension requests must be submitted within 10 days prior to your original due date.


Only one 30-day extension will be granted. No exceptions. If you are unable to submit your video by the extended due date, your account will be made INACTIVE, and the program materials will be removed from MOSSA GO.


When you are ready to prepare and practice for your assessment, you may reactivate your account for a $49 reactivation fee. Once reactivated, you will have 30 days to submit your assessment video. No additional extensions will be granted.


You must complete the entire assessment process, including submitting your video and passing the assessment, within one year of completing your training.









Assessment Extension Fee Required

To complete your request for an extension, you must first purchase an Assessment Extension through the MOSSA Store.

A separate Assessment Extension must be purchased for each program for which you are requesting an extension. When adding the Assessment Extension Fee to your cart, select the quantity needed based on the number of programs requiring an extension.

For example, if you are requesting extensions for two programs, you will need to purchase two Assessment Extension Fees.

Your extension request will not be processed until payment has been completed for all applicable programs.

Completing your online training by the due date allows you to stay on track toward joining your Group Fitness team at your facility and becoming a certified instructor.

If you need additional time, we offer the following extensions for $25 each:

  • 14-day extension for Online Transition Training
  • 30-day extension for Initial Online Training
A maximum of three extensions is permitted.

Extension requests will only be accepted within 10 days of the course due date. Requests submitted outside of this 10-day window will require a $49 reactivation fee, which will count as one of your three available extensions.

Please note: Extensions are granted from the original course due date.
